New and Option Student Registration
NEW STUDENT REGISTRATION
Dear New Students and Parents/Guardians:
If
you have not already done so, you will need to make a registration
appointment by calling the counseling secretary, Faye Winter, at
786-2765. New Student Registration
will be held in August for the new school year and by appointment
only. Registration throughout the school year will be by appointment
only.
Please contact Faye Winter at 402-786-2765 to request a new student packet.
The
packet contains information regarding the registration process for new
students at Waverly High School. There are many forms that must be
completed in order to register; however, completing them prior to your
registration appointment will expedite the process.
1. Please
complete the School District 145 Student Information and Health
Record. This record is used to enter demographic information. It is
critical that contact information be accurate.
2. Complete the
Release of Information form. This form grants us permission to get
records from your previous school. Please note, though, that if YOU
GET THESE RECORDS DIRECTLY FROM YOUR PREVIOUS SCHOOL PRIOR TO YOUR
APPOINTMENT we will be able to fully register you.
IF YOU DO
NOT HAVE RECORDS with you at your appointment, we will not be able to
fully register you or complete your schedule � this may delay your
enrollment in school.
We must receive current and official
transcripts, immunizations, Special Education records (if applicable)
and a birth certificate in order to complete the process. It is in
your best interests to contact your school and either have these
records sent to us directly or bring them with you to your appointment.
3. Carefully review the documents regarding immunizations.
All immunizations must be current and documentation of such must be
shown in order to register. You must also have completed the
Documentation of Chicken Pox (Varicella Disease) form.
4. If
you are in the process of building a home in School District #145, but
are not currently living in the home, you will need to complete the
Affidavit of Residence and Authorization for Enrollment form. This
form must be notarized.
5. Complete the Power of Attorney
form with a notary only if you are a guardian or are currently seeking
guardianship. If you currently have guardianship of your student,
please provide documentation of such at your appointment for placement
in the student�s cumulative file. All guardians must be current
residents of School District #145.
6. If your student
receives Special Education services, you must complete the Permission
for Temporary Placement form. We will also request current IEP and MDT
documents from your previous school; however it would be beneficial for
you to bring these documents to your appointment.
7. You
will need to complete the Viking Activity Guidelines forms in order to
participate in any school activity. The packet also contains a sports
physical form that must completed prior to fall athletic practices. A
list of all school activities has been included as well.
8. A
Course Description booklet is included for you to begin reviewing
course selections. Your choices will be dependent on your current
grade, graduation requirements, and college choices. These will be
reviewed and discussed at your appointment.
9. Complete the Application for Free and Reduced Lunch, if applicable. This form can be turned in during your appointment.
Thank

OPTION ENROLLMENT
Students who do not live within the District #145 boundaries
but are interested in registering at Waverly High School will need to
complete the District's Option Enrollment Form. This form must then be
sent to our Administrative Office for approval at the following address:

NOTE: If class numbers are near capacity, NOT ALL OPTION ENROLLMENT REQUESTS WILL BE GRANTED.
NOTE: In
order to be immediately eligible for Nebraska School Activities
Association (NSAA) sanctioned activities and sports for the 2008-2009
school year, an option enrollment request must be accepted by District
#145 and sent to the NSAA main office by MARCH 15, 2008. Option
Enrollment acceptance beyond that date will result in a student being ineligible
for NSAA activities for 90 days.
