Reimbursement
is an excel spreadsheet to complete for curriculum work in which the
staff member receives a stipend. The form has the account number and
automatically sums the total to be reimbursed. This form should be
completed by the teacher, signed by the building principal, and
forwarded to Toby Boss. The form should be completed immediately after
any ESU 6 or onsite curriculum/assessment work. Word Reimbursement is a word document that can be used by staff members instead of the excel spreadsheet.
Curriculum Map
is a form to record the alignment of the curriculum to state or
national standards and the materials that will be used to deliver the
instruction. Assessment
Analysis is a form to record the methods of collecting the assessment
results, the critical issues and the summary of future plans.
