General Assessments
The following are general forms that can be used for assessment work in any subject area.
Six Quality Criteria is a pdf file with a short description of the criteria.
STARS Reporting schedule is a pdf file with the requirements from 2003-2008
Technical Summary
is a form to complete once the assessment has been reviewed and cut
scores, item difficulty, and reliability have been determined. Range of Difficulty is a form to record the level of difficulty of each question on an assessment.
Item Critique Sheet is a form to record the results of a review of each item on an assessment for Quality Criteria 1-4.
Decision Consistency is a form with the directions of how to use decision consistency to determine reliability.
The following forms are used in the development of an assessment. All three forms should be completed on each assessment.
CRA Task Analysis is a form used to record the skills and knowledge necessary for the standard to be assessed.
CRA Scoring Guide is a summary of the points assigned to each question and the cut scores for each proficiency level.
CRA Administrative Guide includes the specific instructions and materials required to administer an assessment.

- Right click on the desired spreadsheet

- Choose a location to save the file
- Start EXCEL
- Within EXCEL select "File", "Open"
- Select "types of file" = "all files"

- Select "Download link to disk"
- Choose a location to save the file
- Start EXCEL

NOTE If the tabs for different sheets do not show at the bottom of the screen. On the menu bar go to WINDOW, Arrange and mark the box "window of active workbook".
